    Départ du docteur Shayne Baliesh de la Clinique Médicale Santé Plus

    J’ai le regret de vous informer de mon départ de la Clinique Médicale Santé Plus. La date de départ sera le 23 janvier 2025. Je quitterai l’organisation pour pratiquer dans une nouvelle clinique médicale que j’ouvre, qui est également désaffiliée de la RAMQ.

    Si vous désirez poursuivre vos soins à la Clinique Médicale Santé Plus mais avec un autre
    médecin, svp en faire la demande avec la secrétaire et discutera avec l’équipe traitante la possibilité de vous intégrer sous les soins d’un autre médecin.

    Vous êtes également invités à me suivre dans ma nouvelle clinique. Il est toujours en
    construction et la date d’ouverture provisoire sera le 3 février 2025. Il s’appelle Rose M.D. et est situé au Mont-Royal. L’adresse est 250-5500 rue Royalmount, Mont Royal, H4P 1H7. Le site Web contenant de plus amples informations sera disponible dans le mois prochain.

    Departure of doctor Shayne Baliesh from Clinique Médicale Santé Plus

    I regret to inform you of my departure from Clinique Médicale Santé Plus. The date of departure will be January 23, 2025. I will be leaving to practice in a new private clinic that I will be opening, which is also not affiliated with the RAMQ.

    If you would like to continue your care at Clinique Médicale Santé Plus but with another doctor, please ask the secretaries, and they will discuss with the remaining doctors how to integrate our care if possible.

    You are also welcome to follow me at my new clinic. It is still being built, and the
    tentative opening date will be February 3rd, 2025. It is called Rose M.D. and is located in Mont-Royal. The address is 250-5500 Royalmount street, Mont Royal, H4P 1H7.
